- Policy types
We require a verified group administrator or the appointed broker of the policy to request this document when reaching out.
Schedule As can be requested at any time of the year, however, during tax-season these forms may take more time due to increased demand (Q1). Please make sure to give ample time (1-3 Business Days) for the request to be processed.
