Beam recommends pre-treatment estimates prior to completing services.
Documentation Required:
- Please review: Required documentation for claim processing
- Claim must be submitted with current ADA claim form (no claim forms from outside the US are accepted)
- Claim must be submitted in English, using US dollar currency with ADA dental procedure codes.
- These documents should be collected by the member from their provider.
Please note- some procedures require additional documentation such as pre and/or post op xray(s), periodontal charting, narrative, etc.
For Provider reimbursement only:
- For provider to be reimbursed directly, practice must have a US address and tax ID. We have the ability to pay to US remit address if it is a treating foreign location. Please note: payment & allowed fees are calculated based on member's zip code.
- If the practice does not have a US address and tax ID, member should pay provider directly and retain a receipt including procedures completed and all applicable documentation. The member should fill out and submit an ADA claim form including a copy of their receipt with all applicable documentation and submit a claim seeking reimbursement.
- Reimbursement will be based on member's plan, participating provider maximum allowable charge, and/or applicable scheduled fee amounts for the member's zip code.
