To avoid delay in claims processing, please resubmit all claims with a ADA form that is 2006 (or newer) version.

Missing information
- To avoid delays in claims processing, please submit all claims with the following information:
- Both the treating provider's individual NPI (Box 54) and billing provider's NPI (Box 49)
- Provider Tax identification number (Box 51)
- Physical treating location (Box 56)
Incomplete or inaccurate information
- Please ensure that the correct Beam subscriber is submitted on the ADA claim form (Box 12)
