On the claim form, there are two boxes for "Authorizations." Box 36 is always signed because it indicates the patient is responsible for paying the treatment costs if the insurance plan does not cover all or a portion of the claim.
Box 37 authorizes the insurance carrier to direct payment to the dentist instead of to the member. If you want the payment to be sent directly to you, the member, then refrain from signing box 37.
Double check with your provider to make sure they don't automatically fill this box with "Signature on File" and the date.
