How do I submit a critical illness claim with Beam Benefits?
Please note that this process is applicable for critical illness claims administered by Beam Benefits and underwritten by Nationwide Life Insurance Company.
Please follow the below steps to file a critical illness claim:
- Contact Beam Benefits customer support at 1-800-648-1179 if you have questions about filing your claim or need assistance getting started.
- Obtain the Critical Illness Claim Forms. The claim packet includes the Employee Statement, Authorization Form, and Attending Physician's Statement.

- Login to Lighthouse
- Click 'Plan' on the left-hand navigation bar
- Go to the critical illness plan dropdown
- Locate 'Claim Documents' in the 'Documents' section
- Select 'Critical Illness Claim Form'

- Complete the forms. Please be sure to include the completed claim forms and all supporting documents within 90 days after the date of loss (see below for list of items to be submitted). Make sure all required information is provided and all questions are answered completely and accurately — missing or illegible information will delay processing.
- Submit completed claim forms and supporting documentation using one of the following methods:

List of documents to submit:
- Completed and signed Employee Statement.
- If the employee is deceased or incompetent, the surviving spouse or authorized representative should complete the form and provide a copy of the death certificate or Power of Attorney (POA), if applicable.
- Completed and signed Authorization Form. This should be completed by the claimant (employee, spouse, or adult child). If the claimant is a minor child or is incompetent, the claimant's authorized representative should complete and sign the form.
- Completed and signed Attending Physician's Statement along with supporting medical records. This should be completed by the physician treating the critical illness condition.
- Additional documentation may be required based on the condition being claimed.
