- Log in to the Provider Portal
- Go to the Eligibility tab
- Enter the patient's date of birth
- Enter the patient's Social Security Number or Beam Member ID
- Click "Verify"
- Locate the "Patient Docs" column on the righthand side of the screen
- Click "Benefits Summary"
- A new tab will open with the Patient's Benefit Summary document for you to review their benefits coverage information including Plan maxes, In-network and Out-of-Network co-insurances, as well as plan deductibles.
