When you visit your dentist for the first time with Beam Benefits, show them your digital insurance card. You can access your insurance information and your ID card from the Beam mobile app. Login and go to the Insurance tab at the bottom of the screen.
In-network Dentist:
If you go to a dentist in our network, reference the 'Network' section of your insurance card. For a complete list of our networks, visit What networks is Beam in-network with? If they have any questions, have them email us at support@beambenefits.com or call us at 1-800-648-1179.
Out-of-network Dentist:
An out-of-network dentist can still submit your claim to Beam Benefits with the information provided on your digital insurance card. If they decline to submit your claim, download the ADA claim form, complete the form, and email it to us at support@beambenefits.com.

Confirming Coverage:
Your dentist may want to confirm insurance coverage before your appointment, and they can do so by:
- Using the Provider Portal at providers.beambenefits.com
- Using the Fax Back feature by calling 1-800-648-1179, selecting 1, and following the prompts
- Emailing us at support@beambenefits.com
To confirm your coverage, they will need your 10-digit subscriber Beam ID (e.g. 1234-56-7890) or your Social Security Number (SSN), your full legal name, and your date of birth.
