How do I file a short-term or long-term disability claim with The Hartford?
Please note that this process is applicable for both short-term disability (STD) and long-term disability (LTD) claims. Your disability program is managed by The Hartford.
Please follow the below steps to file a disability claim:
- Know when it's time to file a claim. If your absence from work is scheduled (such as an upcoming hospital stay or planned surgery), call The Hartford 30 days prior to your last day of work. If your absence is unscheduled, please call as soon as possible.
- Have the following information ready before you call:
- Your name, address, and other key identification information
- The name of your department and your last full day of active work
- The nature of your claim or leave request
- Your treating physician's name, address, phone number, and fax number
- Your policy number:
- Make the call. Contact The Hartford at 1-888-277-4767, Monday through Friday, 8:00 a.m. – 8:00 p.m. A claims professional will take your information, answer your questions, and file your claim or process your leave request over the phone.
- Stay in touch throughout the process. After your claim is filed, The Hartford may reach out to check on your progress, answer questions, or request additional information. You are also welcome to call them at any time with questions during your leave.
