While Beam Benefits does not currently keep Member's Beneficiary Info on file in our Lighthouse (self-service) system, we do offer complete forms on your policy page to keep that information on file for your member!
Here's how you find your beneficiary forms:
- Log into your account at app.beambenefits.com/groups
- If you have access to multiple companies, use the search bar at the top to find the desired company or group
- Click 'Plans' on the left-hand navigation bar
- Locate the desired plan and select 'See more' for further details, or view/print plan documents
- For Policies that require Beneficiary information to be submitted with claims, a blank form will be available within the document section.
- Supporting documents are available under each plan, including Benefit Summaries and Certificates.
