When a member requests voluntary supplemental life coverage over the Guarantee Issue amount, the member must begin the Evidence of Insurability (EOI) process by completing an online health statement including questions related to their medical history and providing their current height and weight. Once the health questionnaire is completed, the member will receive an immediate response with an approval or denial notification. A denial notification will not provide a reason why the member was denied.
