Please note this process is applicable for hospital indemnity claims.
Please follow the below steps to file a hospital indemnity claim with Beam Benefits:
- Contact our Customer Support team at 1-800-648-1179
- We will send you the Beam Supplemental Hospital Claim Form for you to complete
- Note: This is also available in Lighthouse for HR/benefits administrators who have access to the site.
- Login to Lighthouse
- Click 'Plan' on the left-hand navigation bar
- Go to the hospital indemnity plan dropdown
- Locate 'Claim Documents' in the 'Documents' section
- Select 'Supplemental Hospital Claim Form'
- Note: This is also available in Lighthouse for HR/benefits administrators who have access to the site.
- Complete the form. Please be sure to include the completed claim forms and all supporting documents (see below for list of items to be submitted).
- Submit completed claim forms and supporting documentation using one of the following methods:
